It can be hard but where I work, I try not to assign labels like man or woman to someone until they tell me themselves. We have a couple of people undergoing hormone therapy and while their name may be John they don't seem to like being called "sir" very much.

It's just the world I grew up in and hopefully it's changing for the better but I still can't enter a new patient in as anything other than male, female or unknown. WTF am I supposed to when someone tells me they are neither?

Maybe if it's a medical thing you need to enter the physical gender so the doctor/nurse knows how to deal with the necessities.

I identify as neither (and have medically transitioned as much as is possible) and I love my doctor for actually respecting that. Chromosomes don't matter. The fact I used to have testes doesn't matter.

I think it's totally acceptable to ask that question given the field. When I was filling out job applications I'd always click the "wish not to say" button for race and gender, as it shouldn't make a difference (my name can go both ways) but if it's a physical issue, you sort of need to know. I used to live with a "M2F" who really didn't do much except for stuff and wear girl's clothes and still had a dick, and that's fine to identify as a she in my book. But if I had to take her to a hospital I'd like to think they'd ask. If you just ask the question though it's a party/office etc. foul.

Generally for a medical emergency it's also not actually relevant, unless the emergency is "my dick got cut off" or "my period is bleeding way too much" or "I'm having a miscarriage."

I've spent some time in the ER due to complications from spironolactone when I was still taking that, but that issue is still not inherently gendered (aside from the question of "Why are you taking spironolactone?"). Of course that particular ER was in San Francisco's Castro district so they understood.

It isn't offensive to say if you have a dick you're probably a man and if you have a vulva you're probably a woman, because the vast majority of people so equipped do happen to fit into those categories.

However, it is offensive to paint everyone with those categorical, binary brush strokes, as well as actively harmful to people who are intersex, transitioning, or questioning.
