So... From what I can gather you have to fill up those gauges on the right, which seem to be little symbols representing sperm, a bullet, heart and crucifix. You fill them with pleasure units acquired by running around insano-world doing strange things, for the ultimate goal of becoming a real person. It makes sense and the graphics are good.
